Enterprise Eventbrite - Order Form

IMPORTANT 

For every event, a copy of the community's Order Form will need to be added to the event as part of the event registration process. The home zip code and organization selection within the registration form are both critical to their proper assignment in the database and in HubSpot.

This order form is set to match the BZP Community Portal Landing Page (used for RealAge Test, Live Longer Better Rewards Program, and the Sharecare App).

Organization Leads will input organization drop-down changes via their normal process and the marketing team will update the Order Form in Eventbrite.*

How to find the most recent Order Form

When you create your event you will select: 'Community Name' Order Form and look for the most recent date (if there are any updates to your Order form it will just appear as 'Community Name' Order Form) and add it to your event under Orders | Order Form.

*Please watch the second training video below to learn about how to set up an Organization's Private Event with a form selection. A Private Organization Event would be the only type of event in which you wouldn't use your Order Form.

Please make sure your Order Form is set to EACH ATTENDEE as you create one for a Private Organization event (as shown below).

 

Why is this step critical?

  • HubSpot: As we pass opt-ins into HubSpot, it will be critical that an individual's zip code is included in their registration to provide assignment to your community list.
  • Organization Tracker: For proper organization tracking we need to connect Organization to registrants in Eventbrite.
